The shape is messed up after importing OBJ file into 3DS Max. Was it the correct file?

Thanks for letting me know. I checked the model and found some duplicate mesh in places for no apparent reason. Corrected that and re exported so give it another try and let me know if that didn't fix it. Thanks!

It looks great now. However I wonder why you have many unnecessary edge (lines) that does not impact the polygons, if you removed them? Or did you intend it for textures? Any possibility that you upload the textures too?

I believe I intended to add textures when I first created it but never did. That was so long ago I can't even find my original project file anymore. Sorry.
