Stylized monster mask intended to be used as a simple prop and not much else, because the back is too flat to accommodate a human face and the topology in the low-poly model is too erratic to accommodate smooth animation. It should work hanging on a wall or placed over a minor character's face without clipping. The eye and mouth (with teeth) aren't fused to the mask in the blend file, so they can be easily removed if you wanted to modify this thing into something more usable. The four horns on the sides are not easily removable.
Made in Blender, with the eye texture partly made in MS Paint.
Vertices, faces, and tris:
High-poly: 16,877 V, 18,973 F, 33,678 T
Low-poly: 1,340 V, 2,509 F, 2,604 T
