Lara Croft stuck in a wall

Lara doesn't put away the axes when she jumps so affectively I cannot scramble up to the ledge. The first time this happened was in the middle of the game trying to exit the mirrors tomb. Eventually I gave up and reloaded from the last base camp erasing the whole tomb. I've finished the story now and I want to complete all the challenges but I'm still having this problem.
